New Patient

At Dr. Daniel's office we want to make your smile the best it can be! We believe that orthodontics is not only about straightening our patients’ teeth, but also about making a difference in their lives. At your complimentary first visit, called a new patient exam, Dr. Daniels will do a comprehensive oral exam and talk about diagnosis and treatment options. If necessary, we will take an x-ray at no charge and gladly send it to your dentist. You will also meet our new patient coordinator who will be happy to answer any questions or concerns you may have. We are very committed to our patients and their families.

When your exam is completed you will be provided with an examination summary outlining diagnosis, treatment options, when we recommend beginning treatment and our fees. A copy of this summary will be sent to your family dentist. Working in close contact with your dentist is important to our office so we can achieve the best results possible. After the appointment, Dr. Daniels will suggest either making an appointment to start treatment or revisiting us at a later date. Remember the American Association of Orthodontists recommends visiting the orthodontist at age 7. By age 7, the permanent molars have come in as well as some permanent front teeth. Being a specialist in orthodontics, Dr. Daniels can assess the adult teeth bite relationship and crowding and tell you what treatment (if any) is recommended.

Please be sure to bring a HEALTH HISTORY form to your first appointment!

At the time of the first visit, you can assist us by bringing the following:

  1. Your insurance card if you have insurance so we can verify and/or gather more information
  2. Any panoramic (goes all the way around the head) x-ray taken within the past 6 months
  3. Our health history form, which you can print off our website and fill out (click link)
